Health Inventory – This form needs to be completed by both parents and a pediatrician and must be on file at the center before your child can start.

 

Emergency Card – https://earlychildhood.marylandpublicschools.org/system/files/filedepot/3/occ-1214-emergencyform.pdf – This card needs to be completely filled out.  Take your time and be thorough because this card is used in the event of emergencies.

 

Medication Administration Form – This form must be completed by parents and a physician before GLC can administer medication to your child.
